Book of Recognition

The Book of Recognition is an excellent way to honor an individual for something they have done or in honor of their memory. Funds from the Book of Recognition are used, on vote of the Board of Directors, to promote relations and communication between the Federation and its members.

First District's website will maintain an ongoing list (see below).

Application Process

If you wish to submit an individual for recognition or honor their memory, the required procedure is detailed below in accordance with FGCNYS policy.

 Print and complete the Book of Recognition Form (link below).
Send completed form along with a $10.00 check, made payable to FGCNYS, Inc., to the District Book of Recognition Chair, Suzanne Quinn, or give to Suzanne at a First District monthly meeting.
Suzanne Quinn will send the form and check to the FGCNYS Office. The State will list the person's name on the website. The person who submitted the application will be notified by the state of the posting on the FGCNYS website.
